GIRL SCOUT INFORMATION

ROZE Day Camp is for registered members of Girl Scout of the United States of America. If you have not registered, or did not renew your membership, please contact the Girl Scouts of River Valleys at (800)845-0787 to renew.

Unit Placements and Friend Requests - Girls are placed in Units with other girls of similar age. We attempt to place girls together in Units with a least one Girl Scout friend, if the friend request in mutual and the same grade. Enter up to three friend placement requests below, and we will attempt to match her with at least one of them in her Unit.

Due to logistics and safety ratios we cannot keep entire troops together. ROZE Day Camp is a place to make new friends during the 3 days of camp! Thank you for understanding!

Health Information

Girl Scouts River Valleys requires us to collect the following for girls and volunteers attending camp. The Allergies and Activity Restrictions fields are shared with program staff and her Unit Leadership Team. All other health information is kept confidential with the camp First Aider.

Over-the-Counter Medications: Does the camp nurse have permission to administer (Tylenol (acetaminophen), Advil (ibuprofen), Benadryl (diphenhydramine), Neosporin (antibiotic ointment), Afterbite (sodium bicarbonate/baking soda), Cortizone-10 (hydrocortisone cream), or their generic equivalents, as needed? (Please select Yes or No for each.)

T-Shirts are included for campers, caddies, and volunteers.

Cookie Credits/Fall FUNds

Select the quantity of $5.00 Cookie Credits/Fall FUNds you will redeem. We'll send you a follow-up email to enter the individual electronic codes. You'll receive an invoice or partial refund of the final cookie amount if different than what' indicated below.

Camp Permissions

I give permission for my camper/caddie to attend ROZE Day Camp and participate in all camp activities, unless otherwise indicated under health and activity restrictions. The health information above is complete and accurate. I do not know of any reason(s), other than the information indicated on this form, why my child should not participate in the prescribed activities except as noted. In the event that my child needs medical attention while participating in Girl Scout activities, I authorize the adult in charge to see that my daughter/girl receives routine healthcare, medications, reasonable first aid, and to transport my child to a health care facility for emergency services as needed.

I will NOT send my camper/caddie if she becomes exposed to any contagious illness or diseases including COVID-19. I give permission for my child to be photographed during camp activities for sharing after camp. I will follow camp policies as detail in the confirmation letter and volunteer guides.
